Taking the first step toward understanding your or your loved one’s needs is a big decision, and we’re here to make it as smooth as possible. Follow these easy steps to schedule your appointment, we’ll guide you through the process.


Step One: Complete the New Patient Questionnaire

Before your first visit, you’ll complete a New Patient Questionnaire for yourself or your child. This step helps us get to know your concerns, goals, and background before we meet.

  • The questionnaire will ask for your contact information and ask for a brief description of the presenting concerns.

Completing this step gives us a head start in tailoring our approach to your unique situation.

Step 2: We’ll Contact You to Follow-Up

We will contact you via email and/or phone to discuss scheduling an appointment. Prices for our services will be discussed at this point.

We will send you three pieces of information: a HIPPA form and an authorization to charge a credit/debit card form as well as statement of our office policies. These materials should be completed/signed and brought to the initial interview.

Step 3: Initial Interview

The process begins with a thorough conversation. You, your child, or your family will meet with the psychologist to discuss your concerns and gather vital information. This session is key to forming a clear picture of diagnostic issues.

  • The in-depth interview will focus on presenting concerns.

  • We’ll cover developmental milestones, academic or work history, medical background, and family/social dynamics.

  • You’ll have the opportunity to share any previous evaluations or reports that might help us better understand the situation.

  • During this 60-90 minute meeting, we’ll explain the testing process, gather consent, and provide behavioral questionnaires for parents and teachers.

By the end of this session, we’ll have a roadmap for the assessment and a scheduled testing date.

Step 4: Testing Sessions

This is where we dive deeper to answer the key questions about strengths, challenges, and overall functioning. Testing sessions are tailored to ensure accurate, comfortable participation.

  • Sessions are typically scheduled in the morning when energy levels are highest.

  • Breaks are provided as needed, and children are welcome to bring snacks and drinks for comfort.

  • Parents of infants and toddlers remain present in the testing room for added support.

The testing process gives us the data we need to provide clear answers to your concerns.

Step 5: Feedback Session

Once testing is complete, we carefully analyze the results and prepare an in-depth report. This step is where it all comes together.

  • You’ll receive a comprehensive written report with insights into strengths, areas of need, and recommendations.

  • During the 60-90 minute feedback session, we’ll explain the findings, answer questions, and outline the next steps in detail.

  • The report is designed to be shared with schools, therapists, or other professionals to ensure a coordinated plan moving forward.

You’ll leave with a clear understanding of the results to support growth, development, and success.

  • Absolutely! We are happy to provide you with detailed receipts, including the necessary codes to help you file claims with your insurance provider. However, there is no guarantee of reimbursement.
